ATS Account Creation Instructions- Freshman/Transfer
In lieu of paper forms, the University of the Cumberlands has begun requiring all athletes to fill out all pre-season paperwork online. Please follow the specific instructions below.
Please note: INTERNET EXPLORER is NOT compatible with the ATS system. Please choose a different browser when trying to access the ATS system. (Firefox is preferred; however Google Chrome and Safari should also work as well)
To access the system please go to the following website: ATSUCumberlands2.atsusers.com.
Do not enter anything in the boxes. Click “Forgot Password”. Enter your University of the Cumberlands email address (has to be your UC email-any other email will not work). For Database enter: ATSUCumberlands.
An email will be sent to your UC email address for you to enter a password of your choosing. Please make sure it is something you can remember and not share with anyone else as you are entering private medical information.
Return to: ATSUCumberlands2.atsusers.com and log in with your Athlete ID (the portion of your email before the “@ucumberlands.edu”), your new password and the database of ATSUCumberlands
Once you create your account, you will log into the ATS Athlete portal with the portion of your UC email before the “@ucumberlands.edu” as the Athlete ID and the password as described above for future site visits.
* Please note if this does not work for you, do not panic. We must have your UC email loaded in ATS for it to work. Please contact your supervising athletic trainer listed at the end of these instructions and give them your UC email for them to load so that you can fill out your online medical forms. *
Click on “Athlete Information” and fill out tabs as designated below
General Information Tab
1. All sections are mandatory. Should a question not be applicable to you (i.e. you don’t have allergies to list) please list N/A in that section. Once all required sections are complete click on the “Save Athlete Information” button at the bottom of the page. At this time if you filled anything out incorrectly you will be given a prompt stating what information needs corrected. If no information needs corrected move on to the insurance tab.
2. Please select the team/teams from the drop down menu that you will be on the roster for while at Cumberlands.
3. Fill out the demographics section completely (name, address, email, phone number). Please note the first address you will list is your home address. The additional address below that should be the P.O Box you will have at while at Cumberlands (if you do not know your UC box yet please mark this section with the words “not known”).
4. Please select the year in school you will be in for the upcoming school year from the drop down menu.
5. Under the Medical Alerts, Allergies, and Current Medications sections please either select from the drop down menu or type in the box any pertinent medical information (i.e. asthma, diabetes, sickle cell positive, etc...)
Medical History
1. If surgeries or medical information needs to be added, click the green add button to list any surgeries that you might have had. Also enter any relevant medical information notes in the area provided below. Once all sections are complete, click the “Save Medical History” button at the bottom of the page.
Immunizations
1. Click the green add button to list any immunizations that you have had. Keep in mind, the Tetanus shot is needed every 10 years.
Paperwork Tab
1. Once you complete the forms required under the Forms Tab, they will appear checked off in this area.
Insurance Tab
1. Please enter your primary insurance information. Click the green Add button.
a. Please fill this section out as completely as possible. This helps us later with insurance claims.
b. You will need to enter your insurance information in the column to the left, and the Policy Holder’s Information in the column to the right.
c. Upload pictures of your front and back of your insurance card at the bottom of the page. Note: This is mandatory. IF you do not upload photos of your cards in this area, you can also upload them in the eFiles section, but they MUST be uploaded in one of these two sections. Files are limited to 1MB in size. If you are having difficulty with the size limitation, insert your photos into a word document and upload the word document in the eFiles section.
2. To add additional insurance information:
a. If you have more than one insurance policy please complete step one for each policy. You will need to wait until you complete the General, Insurance, and Contacts tabs completely, and the system saves your information to add a second insurance company in the Insurance tab.
b. Click the green Add button to add a new insurance policy.
c. Please fill out ALL information for your insurance policy as you did above. Once complete please click on the blue SAVE button at the bottom of the page to save the information you have entered.
Contacts Tab (Emergency Contacts)
1. Click the green Add button. Please enter at least two emergency contacts in this area.
a. You may enter multiple contacts if you would like, you are required to have two.
b. The system will only allow you to add one contact initially. You will need to wait until you complete the General, Insurance, and Contacts tabs completely, and the system saves your information to add additional contacts.
2. To add additional Contacts:
a. Click the green Add button to add a new emergency contact.
b. Once the “add new item” box pops up, please fill out ALL information for your emergency contact. Once complete please click on the blue check mark at the bottom of the box to save the information you have entered.
Forms Tab
Please see the list of forms you need to fill out below. You need to complete all forms listed below prior to August 1, 2018.
Once you complete a form it will show the form name, date completed, and any details in the grid at the top of the page.
Forms that must be completed for NEW/TRANSFER athletes:
1. Informed Consent –Acceptance of Risk
2. Consent to Treat
3. Conditioned Authorization for Claims
4. Incoming Health History Form
5. Symptomology Form
6. Drug Testing Form
7. Mandatory Concussion/Staph/ Illness Video (ALL ATHLETES)
8. Mandatory Football Heads Up Video (FOOTBALL PLAYERS ONLY)
9. Agreement to Disclose Injuries or Illnesses
10. Agency Statement
11. UC/NAIA Tobacco, Alcohol, Supplement Statement
To complete a form:
1. Select the Form from the drop down list titled “Form Name”
2. Click on “New” to the right to begin the form (It may take a few seconds to load the entire form)
3. Answer all questions on the form (Some forms only require that you read and sign them)
4. Sign the form in the signature box using your mouse (left click your mouse, while holding the left click button down move your mouse to sign your name in the box). Then type your name in the “signed by “box. Click on the button titled “Sign” next to the box to save your signature (Once your signature is accepted the box will show a messages stating “Document Signed by Athlete/Student). Please note: the Parent/Guardian signature is only required for athletes under the age of 18. If you are over 18 please leave this box.
5. Click “Save” at the bottom of the form. Once the file has been saved and accepted by the system a pop-up stating “Save Complete” will appear on your screen. If you do not see this message your file has not been submitted!! You must sign AND save your form for it to be saved in your account.
6. Continue on to the next form from the drop down box.
Videos
There are two videos available to watch. The Concussion, Staph Infection and Illness Video is a requirement for all UC Athletes. Please follow the link provided in the Mandatory Concussion, Staph Infection and Illness video form. Once you have watched the video, please return to ATS and e-sign the statement that you watched it in its entirety.
The Mandatory Football Heads up Video is only for those athletes that are on the football team (although anyone is free to watch it). Please follow the link provided in the Mandatory Football Heads Up video form. Once you have watched the video, please return to ATS and e-sign the statement that you watched it in its entirety
E-Files Tab
PLEASE NOTE: All files MUST be less than 1MB to upload to the system. Please adjust the resolution on the file, or break the file down into multiple smaller files if necessary to stay under the 1MB file size otherwise your upload will fail (If attempting to send pictures you can also place the pictures in a word document and upload the word document which will save on file space).
1. Any documents requiring scanning and uploading can be uploaded directly to your account from this tab. (if you do not own/have access to a scanner please contact the athletic training staff to determine the best way to get your forms to us)
2. Documents needing uploaded may include, but are not limited to:
a. A copy of your primary insurance card
b. Doctors notes, clearance letters, physical therapy notes, x-ray/MRI reports, etc from previous injuries
c. Single Paired Organ Form (please contact athletic training department to obtain)
3. At the bottom of the eFiles tab, in the box next to “Description” please describe the document you are uploading (i.e. Shoulder clearance note or Insurance Card etc...). Name your file in a way that the sports medicine staff will understand what the document contains.
4. Click “Choose File” to browse your computer for the file you would like to upload. Once you locate the file, click on it and click the “open” button.
5. At the bottom of the eFiles page, click “Upload”. Once the system saves your file a pop-up indicating the save was complete will appear, and you should be able to see your document above under the “Electronic Documents Uploaded by Athlete” heading
If you need to update your information such as address change, new phone number, new insurance information or other data change, you may log in using your username and password at any time. If you have any problems or questions while entering this information, please contact the following or call the athletic training room at 606-539-4369 for assistance.
· You will also need to visit www.ucumberlands.edu/insurance to fill out your online insurance waiver or to enroll in the school offered insurance. You must either have your insurance waiver approved or enroll in the school insurance before being allowed to play intercollegiate athletics at UC.
